FEBRUARY 1, 2008 - New changes in the application process
The U.S. Department of State-Passport Agency has implemented many changes to the process of applying for a U.S. Passport. Be patient with us in Milpitas as we assist you and your family, while following the new regulations. Significant changes include:
- 2 Parent Consent Required for all children age 15 and younger (BOTH PARENTS required to appear with children under age 16)
- New application forms: DS-11 for new passport or child renewal and DS-82 for adult renewal by mail
- New fee schedule - Typical costs (payable by check) are:
Adult passport $75 to US Dept. of State AND
$25 to City of Milpitas
Child Passport $60 to US Dept. of State AND
$25 to City of Milpitas
